NurseAid application form

how to apply

Applications can be made in person, through a Social Worker or other caring agency. Once you have contacted NurseAid for support you will be sent an application form to be completed. Alternatively you can download one here.

As you will see we need to know details of your nursing experience, income and expenditure and what help you are applying for. We will also need a letter supporting your application and quotes/estimates for any purchases or repairs for which you are seeking support. The letter of support for your application should be from someone with a recognized background, this helps us to be sure that the help we give is the most appropriate.

If you have been referred to us by an agency such as Care and Repair, SSAFA or a Citizens Advice Bureau, then the person with whom you have contact there will be able to write a supporting letter. If someone has referred you to NurseAid from your medical team, your GP or Occupational Therapist for example, they can write supporting your application.

If you have applied to us directly then you can ask your GP or Minister of Religion or a Senior Nursing Colleague to support you. If you have any problems with this do not despair – pick up the phone and call NurseAid on 01386 446023 or email at NurseAid and we will be happy to discuss the most appropriate course of action for you.

We ask for details of your income and expenditure as this can help in deciding the level of grant support to offer – also we are sometimes able to offer advice on other sources of funding to which you may be entitled. We do understand that everyone’s situation is different and look at each application individually. If you are applying for funding for a specific item such as a bed or chair we ask for details of the supplier and the cost. If you are seeking help with funding repairs or alterations to your home we ask for copies of quotes for the work to be done. In these situations the grant cheques are made payable to suppliers. Once your completed application is received it will be checked by the Administrator to ensure that all the required information has been given. If there are any omissions or if something is not clear this will be double-checked with you by telephone or post. We do not operate any home visiting system but if you are having real difficulties in completing your form then we may be able to arrange for someone to visit and help. Your application is then put forward to the next meeting of the Grants Committee where a decision will be made. The Grants Committee meets quarterly and decisions are relayed to applicants as quickly as possible after each meeting.

If you are in a crisis situation and need urgent assistance applications can be ‘fast tracked’ and decisions made very quickly. It is always worth picking up the telephone and talking to someone at NurseAid.
